Fix crawl access, indexability, and business accuracy before anything else. If your Google Business Profile or map presence is the main gap, local SEO comes first. If that foundation is sound but your clinic stays absent from commercially relevant AI searches, diagnose those answers directly.
Measure the two separately. A map ranking isn't an AI citation. An AI mention isn't a booked patient.
Local SEO and AI visibility answer different questions
Local SEO for med spas usually starts with Google Maps and the local results inside Google Search. It asks whether the clinic appears when someone searches for a treatment in a defined area.
Google says local results are mainly shaped by relevance, distance, and prominence. A complete Business Profile helps Google understand what the clinic offers. Reviews and links can support prominence. Distance still matters, and no copywriter can optimize it away.
AI visibility asks something else. Does a named assistant mention the clinic when someone asks where to go? Does it describe the clinic accurately? Which pages or public sources does it cite?
That result depends on the platform, prompt, date, and available location context. It isn't a permanent ranking.
| Area | Local SEO | AI visibility |
|---|---|---|
| Primary output | Map and local search position | Mention, description, or citation in a generated answer |
| Context to record | Query, map origin, device, and date | Platform, exact prompt, location context, search mode, and date |
| Useful measures | Local grid movement and Business Profile actions | Share of locked prompts, cited pages, and description accuracy |
| What it doesn't prove | Booked patients or revenue | Medical endorsement, permanent placement, or revenue |
The foundations overlap
This is where the argument gets muddy. Local SEO and AI visibility aren't isolated systems.
Google says its generative AI features use core Search systems. A page must be indexed and eligible for a search snippet before it can appear as a supporting link in AI Overviews or AI Mode. Google also says there's no separate AI file or special schema required.
OpenAI has its own rules. ChatGPT search can rewrite a question into targeted searches and use approximate location. OpenAI says placement isn't guaranteed. A site must allow OAI-SearchBot and traffic from OpenAI's published IP addresses to make its pages eligible for inclusion.
Microsoft reports citations across Copilot and AI-generated Bing answers separately from traditional search metrics. Microsoft also says those citation counts don't represent rankings, clicks, traffic, or the role of a page inside one answer.
The same accurate treatment page may support several surfaces. That doesn't turn every surface into one channel. Our guide to how AI assistants decide which clinics to mention explains that boundary in more detail.
What should your med spa fix first?
Start with the shared foundation when access is broken
If the important pages can't be crawled or indexed, fix that first. Check robots rules, hosting or firewall blocks, canonical tags, index status, and whether important facts exist as readable text.
Accurate structured data can help describe visible facts. It can't replace a useful page, and it doesn't force placement.
Start with local SEO when the Business Profile is the obvious gap
I'd put local SEO first when the profile is unverified, the address or hours are wrong, or the clinic is weak across high-value Maps searches it should plausibly compete for.
Fix the profile and website facts together. Confirm the real clinic name, categories, location, services, and booking route. Then establish a dated local baseline.
An AI citation isn't a substitute for being eligible and accurate where nearby patients already search.
Start with AI visibility when local basics are sound
If the clinic has a healthy local foundation but stays absent from treatment-plus-city prompts, diagnose the assistant result directly.
Lock the prompt. Record the platform and location context. Save the clinics that appeared and inspect the cited sources.
Then fix the smallest evidence gap closest to the decision. That may be a thin treatment page, unclear provider information, or conflicting public details. Treat outside corroboration as a working interpretation to test, not a published OpenAI formula.
Don't assume more content is always the answer. One precise page can be more useful than a batch of generic posts. Serum's ChatGPT SEO work for clinics starts with that diagnosis.
If both are weak, don't run two disconnected campaigns
Start with one shared foundation. Verify the Business Profile. Build or improve the highest-priority treatment and location page. Make the clinic and provider facts consistent.
Then rerun the Maps search and the AI prompt as separate checks. You'll know which surface moved and which one still needs work.
What the current evidence actually supports
A sourced Search Atlas partner artifact reports that a dental campaign improved 82% of its tracked map pins over 20 days. Seventeen more pins entered the top 10, and average position improved by nine.
Serum didn't originate that historical campaign. The artifact doesn't provide booked-patient data, revenue, a complete change log, or a control group. It supports the reported map movement. It doesn't establish a normal timeline.
Serum's own AI visibility case measures something else. An anonymized cosmetic clinic moved from being named in 0 of 9 locked ChatGPT buyer searches on July 2, 2026, to 4 of 9 on August 2.
Three targeted articles were live before the second check. One winning query cited a Serum-produced page directly. That's useful evidence for that result. It still doesn't prove that content caused every change.
The case covered one platform and one monthly reading of each query. It didn't measure consultations, patients, or revenue. Read the full clinic AI visibility case and its limits.
Use the local case as local evidence. Use the ChatGPT case as AI visibility evidence. Don't blend them into one promise.
Measure the outcome you are fixing
For local SEO, record the exact query, search location, date, and map movement. Keep profile calls and clicks separate from rankings.
For AI visibility, record the assistant, prompt, date, location context, whether the clinic was named, and which sources appeared. Repeat the same set on a disclosed cadence.
Bookings and revenue need their own source. Use the CRM, calendar, call tracking, or analytics record. Visibility data can't fill that gap.
The decision rule
If access or business facts are broken, fix the foundation. If Maps is the clear weakness, work on local SEO. If the local foundation is sound but the clinic is missing from defined AI prompts, work on AI visibility for med spas.
Don't buy a content batch before you know which problem you have.
Frequently asked questions
Does local SEO help a med spa appear in AI answers?
It can strengthen the shared foundation. Accurate business information, crawlable pages, and useful treatment content can support search and AI retrieval. They don't guarantee a ChatGPT mention or another AI citation.
Does local SEO now include every AI assistant?
No. Local discovery happens across several products, but their outputs and measurement differ. Track Google Maps, Google AI features, ChatGPT, and Copilot as named surfaces rather than hiding them inside one blended score.
How should a med spa measure an AI visibility result?
Record the assistant, exact prompt, date, location context, whether the clinic was named, and the cited sources. Repeat the same set on a disclosed cadence. Keep bookings and revenue as separate measures.
How quickly can local or AI visibility improve?
There's no honest universal timeline. Starting condition, discovery, indexing, competition, and platform changes all affect the result. Dated case observations shouldn't be presented as a normal timeline or guarantee.
